Referral Process
If you’ve been referred for medical dermatology care, please follow the steps below:
Download and print the referral form.
Complete the patient information section.
Have your family physician complete the referral portion.
Submit the completed form to info@cravebeautylab.com or fax to (587) 461-3154.
Once received, our referral coordinator will contact you to schedule your appointment.
To book a dermatology appointment, please call the clinic.
Medical dermatology appointments are booked by phone to ensure proper scheduling and care.
These appointments are not available through online booking.
